Privacy Policy

We are committed to complying with the terms of the General Data Protection Regulations (GDPR) and the responsible and secure use of your personal data.  We have a legitimate interest in the processing of personal data to provide counselling services. This statement informs you what information we hold, why it is collected, how long it is kept and your rights over your personal data.

Melanie Fellowes is registered with the Information Commissioner's Office (ICO) registration number: ZB564741

Information is collected to provide counselling and psychotherapy services and to assess and manage any potential harm. This information includes:

  • Personal identifier’s (name, date of birth)

  • Contact details (phone number(s), address, email, emergency contact/guardian details) 

  • Relevant background information and risk assessment, where applicable

  • Limited medical history (including GP contact details) 

  • Signed counselling agreement

This information will be kept securely in a locked cabinet and/or a password protected PC or phone. Session notes will be pseudonymised.  Once counselling has finished, information will be held securely for a further five years (in accordance with the terms of our insurance) and then destroyed.

Your personal information will be used only to provide you with our services and to give you information relating to our services. We will not share your personal details with any other person or organisation without your knowledge and permission unless there is a legal requirement, child or adult safeguarding issue or a perceived risk of harm.  

Client work is discussed with a counselling supervisor, but this is under an agreement of confidentiality and anonymity is maintained except for in the event of the above exceptions.

We will take all reasonable precautions to prevent the loss, misuse, or alteration of the information you give us.

Access to your personal information

You can make a ‘subject access request’ under GDPR to find out what personal information is held about you, why it is being held and who it could be disclosed to. We will also let you have a copy of the information. 

To make a request for any personal information held, do this in writing or by email using the contact details above.

What happens next

I will acknowledge your complaint within 30 days of receiving it.  I will then look into your concern and respond to you without undue delay,

If you are not satisfied with my response, you have the right to make a complain to the Information Commissioner’s Office (ICO) at ico.org.uk or by calling 0303 123 1113.
